Job Overview
The Advanced Clinical Pharmacist plays a crucial role in the healthcare team, showcasing expertise in all pharmacist responsibilities, including the interpretation, transcription, and verification of medication orders.

Engagement in daily multidisciplinary rounds is essential, along with conducting thorough drug utilization reviews to advocate for rational medication therapy. This position involves leading and developing clinical services while mentoring and educating team members to ensure precise and appropriate documentation of clinical activities.

Key Responsibilities
- Participate in and lead pharmacy clinical and operational initiatives.
- Monitor patient medication regimens and implement necessary interventions.
- Review, interpret, and enter treatment orders to guarantee dosage accuracy, treatment schedules, routes of administration, and safe delivery based on laboratory data and evidence-based practices.

- Collaborate with various hospital departments to enhance clinical pharmacy functions.
- Develop and lead clinical pharmacy services, including drug consultations, nutritional support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO conversions, renal dosing, and other programs as directed by Pharmacy Leadership.

- Provide comprehensive medication and practice-related education to patients, caregivers, and healthcare professionals.
- Work closely with medical and nursing staff to optimize pharmacotherapeutic aspects of patient care, including infusion and outpatient medication management.

- Offer guidance to staff and patients regarding medication uses, drug interactions, and potential side effects.
- Supervise the preparation of treatments, investigational treatments, prepackaging of medications, and drug returns.
- Maintain accurate records of patient prescriptions, charges, and inventory management.

- Ensure that pharmacy technician responsibilities are fulfilled thoroughly and safely, minimizing risks to patients.
- Analyze and evaluate various courses of action, making independent decisions as necessary.
- Assist in the training of pharmacy students and residents.
- Perform additional duties as assigned.

Qualifications
Education:
A Bachelor's degree in pharmacy or a Doctor of Pharmacy from an ACPE-accredited institution is required.

Experience:
A minimum of 3 years' experience as an Advanced Clinical Pharmacist in a specialized area or at least 5 years of general hospital pharmacy experience is required; completion of a Post Graduate Year 2 (PGY2) pharmacy residency or fellowship is preferred.

Certification/Licensure:
Current state license to practice pharmacy is required, or licensure must be obtained within a specified timeframe upon hire. Basic Life Support certification is required upon hire, and Advanced Cardiac Life Support certification must be obtained within a designated period.
