Overview:

Pharmacists play a crucial role in managing medication therapies, ensuring patient safety, and providing essential drug information. As a level II pharmacist, you will engage in the oversight of pharmacotherapy and quality medication management within designated service areas.

Key Responsibilities:

- Uphold the standards of the OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.

- Operational Duties:
- Accurately prepare and dispense medication orders as per physician directives, adhering to established protocols.
- Interpret and verify drug orders in the computerized medication record system, ensuring accuracy and completeness.
- Compound and dispense various pharmaceutical products, including sterile and non-sterile preparations.
- Manage controlled substances, maintaining compliance with legal and institutional requirements.
- Supervise pharmacy support staff and verify their daily activities.
- Maintain professional competency and current licensure, attending required meetings and completing assessments.
- Analyze medication use processes and contribute to at least one related project annually.
- Provide ambulatory pharmaceutical care services, including drug therapy management and patient education.

- Clinical Duties:
- Ensure safe and effective medication therapies in accordance with established guidelines.
- Monitor patient drug regimens for potential interactions and appropriateness.
- Assist with pharmacokinetics and renal dosing consultations.
- Accurately interpret patient medical records and report any adverse drug reactions promptly.
- Sustain the hospital's drug formulary and promote rational drug therapy selection.
- Offer clinical consultation to healthcare practitioners as needed.
- Deliver timely drug information and education to patients and their families.
- Participate in quality improvement initiatives and medication use reviews.
- Engage in departmental committees and complete designated clinical projects annually.

- Occupational Hazards:
- Potential exposure to hazardous substances and risk of injury if safety protocols are not followed.

Qualifications:

A. Education/Skills:

  • Doctor of Pharmacy or BS Pharmacy degree is required.
  • One of the following is required: BPS Board Certification, Residency, Fellowship, or a minimum of 5 years of hospital experience.

B. Experience:

  • Completion of PGY-1 and PGY-2 residency, Board certification, or 5 years of hospital experience is required at the time of hire.

C. Licenses, Registrations, or Certifications:

  • Pharmacy registration in the state of practice is mandatory.
  • Must obtain state licensure within 60 days of employment.
  • Pharmacist preceptor designation is preferred.
