Overview:
Pharmacists play a crucial role in ensuring the safe and effective use of medications. They are responsible for processing medication orders, overseeing patient drug therapies, and providing essential drug-related information. In addition to standard responsibilities, level II pharmacists are actively involved in managing patient pharmacotherapy and enhancing medication quality across designated service lines.

Operational Responsibilities

  • Accurately prepares and dispenses medication orders as per physician directives, adhering to established protocols.
  • Interprets and verifies medication orders in the computerized patient medication record, ensuring completeness and accuracy.
  • Compounds and dispenses a variety of pharmaceuticals, including sterile and non-sterile products.
  • Manages the issuance of controlled substances and maintains compliance with legal and institutional requirements.
  • Ensures timely and efficient pharmacy services.
  • Supervises pharmacy support staff and verifies their daily activities.
  • Maintains professional competency and current licensure.
  • Completes all required competency assessments and participates in departmental meetings.
  • Engages in medication use analysis and contributes to at least one related project annually.
  • Provides ambulatory pharmaceutical care services, including patient education and medication management.
Clinical Responsibilities

  • Ensures the provision of safe, effective, and economical drug therapies in accordance with established guidelines.
  • Monitors patient drug regimens for potential interactions and appropriateness.
  • Assists with pharmacokinetics consultations and renal dosing as per hospital protocols.
  • Accurately interprets patient medical records and reports any adverse drug reactions promptly.
  • Supports the hospital's drug formulary and promotes rational drug therapy selection.
  • Provides clinical consultation to healthcare practitioners as needed.
  • Delivers timely and accurate drug information to the healthcare team.
  • Educates patients and their families about medication use and management.
  • Participates in quality improvement initiatives and medication use reviews.
  • Conducts clinical interventions as defined by departmental protocols.
  • Engages in stewardship activities for antibiotics and opioids as assigned.
  • Attends interdisciplinary rounds as required.
  • Provides services efficiently and effectively.
Contributions to the Pharmacy Department

  • Mentors pharmacy residents and students.
  • Evaluates ongoing pharmacist competencies.
  • Conducts in-service training for pharmacy and nursing staff.
  • Participates in departmental committees as assigned.
  • Completes at least one clinical project annually.

Qualifications:
A. Education/Skills: Doctor of Pharmacy or BS Pharmacy degree is required.

One of the following is required:
BPS Board Certification, Residency, Fellowship, or a minimum of 5 years of hospital experience.
B. Experience: Completion of PGY-1 and PGY-2 residency, Board certification, or 5 years of hospital experience is required at the time of hire.
C. Licenses, Registrations, or Certifications: Must hold pharmacy registration in the state of practice and obtain state licensure within 60 days of employment. Pharmacist preceptor status is required within 90 days. BLS or ACLS and PALS certifications must be obtained within 180 days of hire.
