Position Overview: As a Medication Management Specialist, you will be instrumental in our goal to enhance patient care by optimizing medication therapies. Collaborating closely with pharmacists, healthcare providers, and patients, you will ensure that medication plans are personalized to meet each patient's unique requirements. Your meticulous attention to detail, compassion, and pharmaceutical knowledge will be vital in supporting patients in achieving their health objectives.

Key Responsibilities:

  1. Medication Assessment and Customization: Perform comprehensive reviews of patient medications, taking into account their medical histories, allergies, and specific needs. Work in partnership with pharmacists and healthcare professionals to customize medication regimens.
  2. Therapeutic Management: Monitor and evaluate patients' responses to their medications, providing insights and recommendations for modifications or alternatives to ensure optimal therapeutic results.
  3. Patient Instruction: Provide education to patients regarding their medication plans, including dosage, administration, and potential side effects, empowering them to manage their health effectively.
  4. Specialized Compounding: Prepare specialized medications, including compounding and tailored formulations, to address specific patient needs.
  5. Adherence Support: Evaluate and enhance patient adherence to medication regimens through counseling, reminders, and follow-up communications.
  6. Inventory Oversight: Assist in managing pharmacy inventory, ensuring the availability of essential medications and supplies.

Qualifications:
  • High school diploma or equivalent; Pharmacy Technician certification is preferred.
  • State licensure as a Pharmacy Technician.
  • Strong attention to detail and excellent organizational abilities.
  • Outstanding interpersonal and communication skills.
  • Knowledge of pharmaceutical principles and practices.
  • Ability to adapt to new technologies and pharmacy software.
