Job Overview

We are seeking a dedicated and compassionate Pediatric Home Health Nurse to join our team at Thrive Skilled Pediatric Care. Key Responsibilities
  • Provide one-on-one care to pediatric patients in their homes, promoting their physical, emotional, and developmental well-being.
  • Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Administer medications, perform medical procedures, and provide wound care, as needed.
  • Conduct thorough assessments, documenting patient progress and identifying areas for improvement.
  • Collaborate with our Social Responsibility Committee and Thrive Cares Foundation to address the social determinants of health and promote patient advocacy.
