Clients collaborate with a range of specialists, including physical, occupational, cognitive, behavioral, and speech therapists, as well as Transitional Living Services staff, to acquire essential independent living skills necessary for community engagement.

Job Summary


Registered Physical Therapists (RPT) employ a rehabilitative strategy aimed at helping individuals attain their maximum potential in motor functioning and mobility.

The RPT collaborates with clients to establish tailored therapy objectives that focus on enhancing their motor skills and mobility.

RPTs are responsible for overseeing the work of physical therapy assistants.

Responsibilities and Duties

- Embrace and implement the core principles of Minds Matter, LLC in all service delivery aspects.

- Review, co-sign, and provide constructive feedback on the daily documentation of assistant therapists.

- Assist clients in identifying their needs and developing effective compensation strategies to address the impacts of their brain injuries, offering innovative solutions for daily activities.


- Conduct evaluations that assess clients' physical and cognitive capabilities and analyze medical information to determine optimal therapeutic methods.

- Complete necessary documentation as outlined in each client's care plan (e.g., evaluations, progress notes, daily notes, letters of medical necessity, etc.)

- Integrate employment, educational, and independent living objectives into therapy sessions.

- Collaborate with clients and interdisciplinary staff to monitor progress towards established goals.

- Participate in team meetings and assist clients in setting their objectives.

- Provide services strictly as specified in each client's authorized care plan.

- Assess and recommend adaptive equipment and/or home modifications, as well as strategies for enhancing home and community environments.

- Work directly with clients to enhance their skills across all areas of their life goals.

- Document therapy sessions on the same day they occur, ensuring the notes are goal-oriented, concise, factual, and clear.


- Educate and support families and caregivers regarding the implications of brain injuries, strategies, and techniques, while maintaining open communication about goals and progress.


- Attend all required training and workshops to ensure the delivery of the most current, effective, and evidence-based practices.


- Establish and nurture professional relationships with all clients, colleagues, referral agents, and representatives from other community organizations and services.

- Promote positive public relations for the program within the agency and the broader community.

- Demonstrate a willingness to participate in agency and interagency task groups as requested.

- Perform additional duties as assigned by supervisors.

- Maintain care and functionality of Minds Matter LLC issued devices.

- Uphold confidentiality of protected health information in compliance with HIPAA regulations.


- Potentially serve as a team lead for clients, which includes being responsible for updating goal plans and documenting team meeting histories, addressing progress, strategy effectiveness, concerns, task assignments, or changes to goals.

Qualifications and Skills

- Ability to instruct individuals with disabilities.

- Effectively supervise and mentor therapy assistants, providing timely feedback and guidance.

- High sensitivity towards all individuals.

- Professional demeanor when interacting on a one-on-one basis.

- Strong planning, coordination, and assistance skills regarding all aspects of a client's life.

- Excellent time management abilities, transitioning smoothly between tasks without losing efficiency or composure.

- Creativity in problem-solving.

- Strong verbal and written communication skills.

- Ability to meet deadlines consistently.

- Basic to moderate proficiency with technology, particularly with our electronic medical record system operated on an iPad.

Education and Experience

- Successful completion of a physical therapy program from an accredited institution. Must hold a current license in the state of Kansas.

Requirements

- Some lifting may be required for clients with physical needs.

- Strong verbal communication and listening skills are essential.

- Ability to read and write proficiently.

- Basic proficiency with iPad technology.

- Reliable transportation is necessary.

- Regularly check voicemail and email for communication.
