Responsibilities

  • Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare teams to address unique needs and goals.
  • Administer medications, perform medical procedures, and provide wound care, as needed.
  • Monitor patients' conditions, reporting any changes or concerns to the healthcare team.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and comprehensive support.
Requirements
  • RN/LPN with a current, valid license to practice as issued by the State Board of Nursing.
  • Current CPR certification.
