RN - Extended Care/Transitional Care Specialist

We are seeking a compassionate Registered Nurse to join our team at Providence Extended Care in Anchorage, AK. As a Primary Care Nurse, you will work collaboratively with a Cottage nursing team to provide high-quality care to our elderly patients.
Responsibilities- Administer medications and treatments, assist with care and interventions, and supervise team members.
- Monitor the implementation of care plans and coordinate referrals to other members of the interdisciplinary healthcare team, families, and physicians.
- Promote a resident-directed care model in the cottage setting, utilizing every aspect of the environment and care model to promote quality of life and a sense of belonging.
