About the Role

The Primary Care Nurse (PCN) plays a vital role in the Cottage nursing team, providing and supervising resident care for a specific group of residents. This includes administering medications and treatments, assisting with care and interventions, life engagement, supervision of team members, monitoring implementation of the care plan, and coordinating referrals to other members of the interdisciplinary healthcare team, families, and physicians.

The PCN promotes a resident-directed care model in the cottage setting, utilizing every aspect of the environment and care model to promote quality of life and a sense of belonging.

Responsibilities
  • Administer medications and treatments as prescribed
  • Assist with care and interventions, including life engagement and supervision of team members
  • Monitor implementation of the care plan and coordinate referrals to other members of the interdisciplinary healthcare team
  • Supervise and support team members in providing high-quality care
  • Collaborate with families and physicians to ensure comprehensive care
Requirements
  • Graduate of an accredited School of Nursing
  • Alaska Registered Nurse License
  • National Provider BLS - American Heart Association
  • 6 months Nursing experience in a healthcare facility (acute care, long-term care, rehabilitation center)
  • Supervision/leadership experience
