RN - Extended Care/Transitional Care Specialist

Providence Extended Care in Anchorage, AK is seeking a compassionate Registered Nurse (RN) to provide high-quality care to our elderly patients. As a member of our interdisciplinary healthcare team, you will work collaboratively to deliver exceptional patient outcomes and promote a resident-directed care model.
About the Role:- Provide direct patient care, including administering medications, assisting with daily living activities, and supervising care teams.
- Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
- Monitor patient progress, identify areas for improvement, and make recommendations for enhanced care.
- Participate in quality improvement initiatives and contribute to a culture of excellence.

- National Provider BLS - American Heart Association.
