RN - Extended Care/Transitional Care Specialist

We are seeking a compassionate Registered Nurse to care for our elderly patients in a unique and supportive environment. As a member of our Cottage nursing team, you will provide high-quality care and supervision to residents with varying acuity levels, including those with tracheostomies, dementia, and other complex conditions.
Key Responsibilities- Administer medications and treatments, assist with care and interventions, and promote a resident-directed care model
- Supervise team members, monitor implementation of care plans, and coordinate referrals to other healthcare team members and families
- Work collaboratively with the interdisciplinary team to provide comprehensive care and support to residents
- Graduate of an accredited School of Nursing
- Alaska Registered Nurse License upon hire
- National Provider BLS - American Heart Association within 30 days of hire
- 6 months of nursing experience in a healthcare facility (acute care, long-term care, rehabilitation center)
- Supervision and leadership experience
