ROLE SUMMARY

The Housing Support Specialist will deliver comprehensive, community-based case management services to individuals and families transitioning from homelessness. This role involves utilizing proven methodologies to provide intensive case management, conduct assessments, develop service plans, connect clients to resources, and offer crisis intervention for clients facing multifaceted challenges, including medical, mental health, and substance use issues. Collaboration with local organizations and service providers is essential to assess housing stability and deliver tailored support services.

KEY RESPONSIBILITIES

  • Build trusting relationships with clients transitioning from homelessness using Trauma-Informed Care, Harm Reduction, and Housing First principles.
  • Deliver intensive case management services that encompass independent living skills, housing stabilization, financial management, community integration, employment assistance, and access to healthcare services.
  • Work with clients to ensure they have a thorough biopsychosocial assessment, developing personalized goal plans that are reviewed and updated regularly.
  • Maintain accurate client records and documentation in compliance with program standards and organizational policies.
  • Provide crisis prevention and intervention strategies, including safety planning and de-escalation techniques.
  • Advocate for clients to resolve issues with landlords and facilitate access to eviction prevention resources.
  • Engage in ongoing professional development and stay informed about best practices in the field.
  • Participate in case conferences, team meetings, and community outreach efforts as required.
  • Complete additional tasks and responsibilities as assigned by management.

ADDITIONAL DUTIES:

  • Deliver field-based support services, which may involve travel to various locations to assist clients.
  • Connect clients with additional resources, including supportive housing options.

QUALIFICATIONS:

  • Understanding of the challenges faced by individuals experiencing homelessness, including health and legal issues.
  • Familiarity with best practices in homeless services, such as Housing First and Trauma-Informed Care.
  • Ability to engage empathetically with clients from diverse backgrounds.
  • Strong advocacy and conflict resolution skills.
  • Proficient in electronic data management and documentation.
  • Excellent communication skills, both written and verbal.
  • Bilingual proficiency in Spanish is a plus.

WORK ENVIRONMENT:

This position requires flexibility in scheduling, including potential evening and weekend hours. Regular travel within the community is necessary, and candidates must possess a valid driver's license and maintain appropriate insurance.
