ROLE SUMMARY
The Housing Support Specialist will deliver comprehensive, community-based case management services to households with medium to high needs transitioning from homelessness. This role will focus on intensive case management, conducting assessments, creating personalized service plans, connecting clients to resources, and providing crisis intervention for clients facing various challenges, including medical, mental health, and substance use issues. The specialist will work closely with local organizations and service providers to ensure clients receive the support necessary for housing stability and overall well-being.
KEY RESPONSIBILITIES
- Build trusting relationships with clients transitioning from homelessness using Trauma-Informed Care, Harm Reduction, and Housing First approaches.
- Deliver intensive case management services, including independent living skills training, housing stabilization, financial management, community integration, employment support, and access to healthcare services.
- Collaborate with clients to conduct thorough biopsychosocial assessments and develop individualized goal plans, reviewing and updating them regularly.
- Maintain accurate client records in compliance with agency policies and documentation standards.
- Provide crisis prevention and intervention strategies, ensuring safety planning and de-escalation support for clients.
- Advocate for clients to resolve issues with landlords and facilitate access to eviction prevention and fair housing resources.
- Engage in ongoing professional development and training to stay informed about best practices in homeless services.
- Participate in team meetings, case conferences, and community outreach as required.
- Perform additional duties as assigned by the supervisor.
ADDITIONAL DUTIES:
- Provide field-based support services, requiring travel throughout the community to connect clients with necessary resources.
- Assist clients with high acuity needs by linking them to supportive housing options.
QUALIFICATIONS:
- Experience and knowledge of challenges faced by individuals experiencing homelessness, including health, substance abuse, and mental health issues.
- Familiarity with best practices in homeless services, such as Housing First and Trauma-Informed Care.
- Strong interpersonal skills with the ability to engage clients empathetically.
- Adaptability to work in dynamic environments, including office, home, and field settings.
- Excellent communication, advocacy, and conflict resolution skills.
- Proficiency in electronic data management and Microsoft Office applications.
- Bilingual proficiency in Spanish is a plus.
OTHER REQUIREMENTS:
- Ability to work flexible hours, including evenings and weekends as needed.
- Maintain CPR/First Aid Certification and comply with health clearance requirements.
- Possess a valid driver's license and maintain auto insurance.
- Adhere to confidentiality standards in accordance with HIPAA regulations.
