Position Title: Case Manager (Veteran Support Services)

Location: Lehigh/Northampton (In-Office)

FLSA Status: Non-Exempt

Working Schedule: Monday to Friday (8 AM to 4 PM); Occasional evenings and weekends may be necessary.

Employment Type: Full Time; 37.5 hours/week

Reports To: Assistant Director for Community Services

Position Overview: The Case Manager for Veteran Support Services will operate under the guidance of the Program Director and Assistant Director for Community Services. This role is pivotal in assisting veteran families navigate the housing stability process, linking them to essential resources and providing direct support as required, with the ultimate aim of securing stable housing for veterans facing homelessness or at risk of losing their residences.

The ideal candidate will be detail-oriented, organized, and possess strong client engagement and motivational abilities. Proficiency in multitasking, comfort with MS Office and data entry systems, including HMIS, and exceptional verbal and written communication skills are essential.

Key Responsibilities:

  • Conduct respectful and thorough intake and assessment of assistance requests.
  • Process assistance requests promptly in alignment with organizational policies and grant stipulations.
  • Maintain accurate client records in compliance with organizational policies and grant requirements.
  • Develop and execute a comprehensive Service Plan.
  • Prepare and submit written reports on a weekly, monthly, quarterly, and annual basis as required.
  • Provide information and referrals to additional internal and external support services, including employment resources.
  • Stay informed about current community social needs and relevant government regulations.
  • Represent the organization on community-based committees or task forces as assigned.
  • Deliver presentations at public events as needed.
  • Supervise interns or students as assigned.
  • Collaborate effectively with team members on shared projects and initiatives.
  • Perform other duties as assigned.

Minimum Educational Requirements:

  • Bachelor's degree in Social Work or a related field is required. A minimum of two years in a frontline human services role is preferred.

Essential Knowledge and Skills:

  • Employees are expected to uphold respect for the teachings and discipline of the Catholic Church.
  • Proficiency in oral and written English is required; proficiency in Spanish is advantageous.
  • Strong customer service skills are essential.
  • Well-developed computer skills, including proficiency in Outlook, Word, and Excel, as well as the ability to learn specific web-based applications.
  • Ability to thrive in a fast-paced environment, working collaboratively within a team and managing priorities effectively.
  • Attention to detail is crucial.

Physical Requirements:

  • Employees are regularly required to sit, walk, stand, speak, or hear, both in person and by telephone. Use of hands to operate standard office equipment is necessary, with occasional lifting of up to ten pounds.
  • Employees must be able to drive to various locations as required.

Work Environment:

  • General office setting with some community-based work.
  • Willingness to travel within the designated service area and attend conferences or training sessions as required.
  • Demonstrate professionalism and respect in all client interactions.
  • Normal working hours are Monday through Friday, 8:00 AM to 4:00 PM, with occasional evening work as needed.

Clearances and Training:

  • All required clearances must be obtained prior to employment, including background checks and mandated reporter training.
  • Completion of necessary training programs is required before the start date and periodically thereafter.
