Key Responsibilities:
  • Manage assessment scheduling and ensure the completion of the MDS in compliance with RAI guidelines and regulations.
  • Conduct case mix analysis and provide insights to the Regional Director of Resident Assessment Services.
  • Ensure adherence to Federal Medicare and PA Medicaid Guidelines in all processes.
  • Facilitate care conference meetings in accordance with regulatory requirements.
  • Review Quality Measures (QM) and Quality Reporting Program (QRP) reports, participating in discussions to enhance care quality.
  • Engage in the triple check process to maintain accuracy and compliance.

Qualifications:
Education:
Must be a graduate from an accredited nursing program and hold a valid nursing license in Pennsylvania as a Registered Nurse or Licensed Practical Nurse.

Experience:
Prior experience as an MDS Coordinator is preferred.

Work Schedule:
Monday to Friday, with no weekend or holiday shifts required.
