Job SummaryWe are seeking a skilled and compassionate Hospice Registered Nurse to join our team. As a Hospice Registered Nurse, you will play a vital role in providing end-of-life care to our patients and their families. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide high-quality, patient-centered care to patients and their families
- Develop and implement individualized care plans that meet the unique needs of each patient
- Collaborate with the interdisciplinary team to ensure seamless care coordination
- Conduct regular assessments and evaluations to ensure patient comfort and well-being
- Communicate effectively with patients, families, and healthcare team members
- Current RN licensure in the state of Texas
- Minimum of 1 year of experience in hospice care or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
