Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will provide intermittent skilled nursing services to patients, communicating their progress with other disciplines and directing, supervising, and instructing nonprofessional hospice aide staff in the provision of personal care.

Responsibilities
  • Admission and Assessment: Admit patients eligible for hospice services under the physician's order, assess and evaluate patient needs and problems, and identify mutually agreed-upon goals with patients.
  • Communication and Collaboration: Report patient status and need for other disciplines to clinical leadership, attending physician, and hospice physician, and update care plans on an ongoing basis.
  • Patient Care and Support: Provide intermittent skilled nursing services, including informational visits and obtaining patient consents for hospice admission, and ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care.
Qualifications
  • Education and Licensure: Current unencumbered registered nurse in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
  • Experience and Skills: Ability to work in a field setting, make sound nursing judgments, assess patient needs, and formulate individualized patient care plans to meet those needs.
