About the Role:

Gentiva is seeking a compassionate and skilled Hospice Certified Nursing Assistant to join our team. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support.

Key Responsibilities:

  • Assist patients with daily living activities, such as bathing, dressing, and grooming, to promote independence and dignity.
  • Monitor patients' vital signs and report any changes to the nurse to ensure timely interventions.
  • Administer medication as directed by the nurse, adhering to established protocols and guidelines.
  • Provide emotional support and companionship to patients and their families, fostering a sense of comfort and well-being.
  • Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements.
  • Collaborate with the interdisciplinary team to develop and implement comprehensive care plans, addressing patients' physical, emotional, and spiritual needs.

Requirements:

  • Current certification as a Hospice Certified Nursing Assistant (HCNA)
  • Experience working in a hospice or palliative care setting
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
  • Strong attention to detail and organizational skills
