Job Title:
Long Term Services and Supports Care Manager

Location:
Remote position with occasional visits to clients in various settings. The role involves significant member interaction, with approximately 80% of the time spent in direct engagement.

Preferred Language:

Contract Duration:
12 months, with the possibility of extension.

Working Hours:
8 AM to 5 PM, Monday to Friday.

Key Responsibilities:

Overseeing a caseload of healthcare clients requiring long-term care services. Conducting monthly and quarterly follow-ups with clients, necessitating travel. A valid driver's license is required. Performing comprehensive assessments and documenting client interactions. Collaborating with clients, caregivers, and service providers to gather essential information for care plan formulation. Monitoring service delivery and maintaining contact with clients and their support systems through in-person visits and phone calls. Authorizing and coordinating referrals for necessary services. Ensuring continuity of care by identifying and addressing any gaps in service delivery. Assisting in the integration of informal or voluntary services into client care plans. Working alongside discharge planners, healthcare providers, and other stakeholders to facilitate effective discharge and care planning. Aiding clients in navigating and resolving complaints and appeals.
Educational Requirements:

A Bachelor's degree or a Registered Nurse License with a minimum of 2 years of experience in care management, or a Licensed Practical Nurse with at least 4 years of care management experience, or 6 years of relevant care management experience.

Experience in home health, discharge planning, or long-term care is preferred.

Licenses/Certifications:
A valid driver's license is mandatory.
