Clinical Manager

Pinnacle Home Care Inc. is seeking an experienced and skilled Clinical Manager to join our team. As a Clinical Manager, you will be responsible for ensuring that patient care is coordinated and managed appropriately. You will oversee the delivery of quality home care services and supervise clinical personnel to ensure that patients receive the best possible care.
Key Responsibilities- Case Management: Receive case referrals, review patient information, and assign clinicians to cases.
- Clinical Oversight: Review and evaluate each case, provide guidance to clinicians, and promote effective performance and delivery of quality home care services.
- Patient Care: Review patient's clinical diagnosis, medications, procedures, and clinical course.
- Therapeutic Goals: Assist clinicians in establishing immediate and long-term therapeutic goals, setting priorities, and developing the plan of care.
- Team Leadership: Lead the team by Agency Mission, Vision, and Values, and drive operations aligned with our Sacred Seven.
- Compliance: Consistently follow and uniformly enforce Agency policies and procedures, and maintain compliance with all Standard Operating Procedures.
- Education and Training: Perform consistent 1:1 meetings with clinicians, provide monthly SHP scorecards, and continued education.
- Field Visits: Perform field visits with clinicians to mentor and support clinical excellence consistently.
- Technology: Promote the use of the patient portal, physician portal, and Synzi, and promote Telehealth visits to support appropriate visit utilization.
- Education: Registered nurse with current licensure to practice professional nursing in the state, and a BSN degree is preferred.
- Experience: Two years of management or supervisory experience in a health care setting, preferably home care.
- Certifications: Complies with accepted professional standards and practice, and maintains current CPR certification.
- Skills: Demonstrates excellent observation, organization, and verbal and written communication skills, and is proficient in Computers, Microsoft, EMR, and Technology.
- Knowledge: Knowledge of governmental regulations and ACHC standards.
