Key Responsibilities- Department Management: Manage and direct the Social Services Department, ensuring all medically related emotional and social needs of patients are met in accordance with applicable laws, regulations, and standards.
- Assessment and Intervention: Understand the MDS process, interview, input, and complete social service assessments, accurately assess, document, and provide interventions to meet psychosocial needs of all residents.
- Care Planning: Develop care plans based on assessments, ensuring the delivery of high-quality social services to our patients.
- Collaboration and Communication: Maintain a quality working relationship with the medical profession and other health-related facilities and organizations, facilitating effective communication and collaboration to ensure seamless care delivery.
- Administrative Requirements: Perform administrative tasks, such as completing necessary forms, reports, and submitting them to the Administrator as required.
- Resident Care Conferences: Facilitate resident care conferences, scheduling, sending letters to families/responsible parties, and participating in care conferences.
- Discharge Planning: Facilitate all resident discharges, arranging for home health, DME, and assisting with transfers to other facilities.
