About the Role

We are seeking a highly skilled Licensed Clinical Social Worker to join our team as a Medical Care Manager at Drift Services. As a key member of our healthcare team, you will play a vital role in improving health care outcomes, supporting clients, and addressing barriers to care and treatment.

Key Responsibilities
  • Provide brief interventions focused on long-term achievable goals, working collaboratively with clients to achieve viral suppression, better health outcomes, and quality of life.
  • Build rapport, provide supportive listening, and ensure coordination of services to meet the unique needs of People Living with HIV (PLWH).
  • Work with providers and the healthcare center team to ensure every Ryan White patient receives the support they require and access to available resources.
  • Complete Comprehensive Assessments, develop Individualized Care Plans, and provide ongoing education to patients to ensure they understand their diagnoses and medications.
  • Collaborate with clients to address mental health conditions or substance abuse patterns that interfere with their ability to adhere to medical/medication regimens.
Requirements
  • LMSW or LCSW required.
  • Current and good standing licensure as a Bachelor or Master's Social Worker in the state of Florida.
Preferred Qualifications
  • Bilingual in Spanish preferred.
  • 1 Year minimum of Case Management experience preferred.
  • Knowledge of Ryan White Services preferred.
