Job Summary

We are seeking an experienced Medical Records Coordinator to join our team at Liberty Healthcare & Rehabilitation Services. As a Medical Records Coordinator, you will play a critical role in maintaining accurate and up-to-date patient records, ensuring compliance with regulatory requirements, and supporting the delivery of high-quality patient care.

Key Responsibilities
  • Maintain Accurate Patient Records: Ensure that patient records are complete, accurate, and up-to-date, including demographic information, medical history, and treatment plans.
  • Coordinate Clinical Data Management: Oversee the collection, analysis, and reporting of clinical data, including patient outcomes, quality metrics, and regulatory compliance.
  • Support Interdisciplinary Care Planning: Collaborate with healthcare teams to develop and implement patient care plans, ensuring that patient needs are met and care is delivered in a timely and effective manner.
  • Ensure Regulatory Compliance: Maintain knowledge of relevant laws, regulations, and industry standards, ensuring that patient records and clinical data are managed in accordance with these requirements.
  • Provide Excellent Customer Service: Respond to inquiries from patients, families, and healthcare teams in a timely and professional manner, providing accurate and helpful information.
Requirements
  • Registered Nurse (RN) License: Current, valid RN license from the North Carolina Board of Nursing and proof of license renewal every other year.
  • Experience with Clinical Data Management: Experience with MDS-RAP and Care Planning functions, with a preference for experience with RUG-IV and MDS 3.0.
  • Strong Communication and Organizational Skills: Ability to read, know, and follow personnel, department, and facility policies and procedures, and adhere to local, state, and federal requirements.
