Position Summary:

The Healthcare Services Coordinator will serve as a vital resource for clients and their families, providing essential information and clarifications regarding organizational protocols and guidelines. This role encompasses comprehensive medical case management, referral services, eligibility assessments, and crisis intervention for adults receiving medical care in the outpatient Infectious Diseases Clinic.

The Coordinator will conduct thorough evaluations, identify service requirements, and formulate care strategies, ensuring seamless access to services within the clinic and the broader community. Responsibilities may also include assisting clients with transportation and housing challenges, as well as guiding them through financial assistance programs.

Collaboration is key; the Coordinator will work in conjunction with a licensed social work team, medical professionals, nursing staff, and benefits coordinators as part of an interdisciplinary approach. This position demands meticulous documentation and data entry within electronic medical records and tracking systems. Successful candidates will demonstrate exceptional multitasking abilities in a dynamic environment and are expected to participate in training sessions and meetings as required.

Minimum Education and Experience Requirements:

A Bachelor's degree in a Human Services field with relevant coursework is required. Certain roles may necessitate certification as a licensed Clinical Social Worker as per state regulations.

Required Qualifications, Competencies, and Experience:

Candidates should possess coursework and experience pertinent to the specific area of healthcare services.

Preferred Qualifications, Competencies, and Experience:

Certification in case management is highly desirable. Experience with clinic populations, electronic medical records, and data management is preferred.

Position/Schedule Requirements:

Clinic-based role with occasional overtime.
