Position Summary:
Duke University Health System is seeking a qualified Organ Transplant Coordinator who will align with our mission of Advancing Health Together.

Department Overview:
The Pre-Liver Transplant Coordinator plays a vital role in delivering exceptional care to ensure an outstanding patient experience. This position involves direct patient interaction, participating in daily transplant rounds with the interdisciplinary team to address clinical, psychosocial, and educational concerns for post-transplant patients.

Key Responsibilities:
- Educate patients and their families on all aspects of post-transplant care, including scheduling follow-up appointments and managing laboratory tests.
- Collaborate with the multidisciplinary team to address specific patient needs, such as outpatient pulmonary rehabilitation, IV therapies, and medication management.
- Communicate effectively with patients and caregivers regarding recovery processes and facilitate discharge planning.
- Prepare written summaries of transplant education to be sent to the post-transplant coordinator, documenting educational activities in the Electronic Medical Record.
- Conduct follow-up calls within 24-48 hours post-discharge to review education, discuss future appointments, and address any concerns.
- Document inpatient care summaries for the post-transplant coordinator, detailing care provided and any modifications to the care plan.
- Utilize clinical judgment based on current research and principles to resolve complex issues.
- Collaborate with medical faculty, pharmacy, nursing staff, case management, and other health professionals to meet patient and family needs.
- Stay informed on post-transplant research and apply evidence-based recommendations to patient care.
- Complete regulatory documentation in the UNOS database and hospital EMR, reviewing educational materials and collaborating with the department on practice updates.

Work Environment:
This role requires shared on-call responsibilities within a large team.

Qualifications:
- Education: BSN required; MSN preferred.
- Experience: Minimum of 5 years nursing experience, with at least 2 years in transplant, critical care, or chronic disease settings.
- Licensure: Current or compact RN licensure in North Carolina is required; BLS certification is also required.
