Position Overview:

The Clinical Care Coordinator will serve as a vital resource for clients and their families, providing essential information and clarifications regarding organizational policies and procedures. This role involves comprehensive medical case management, including referrals, eligibility assessments, and crisis intervention for adults with HIV receiving outpatient services. Key responsibilities include:

  • Conducting thorough assessments to identify client needs.
  • Developing and implementing individualized care plans.
  • Facilitating access to services within the clinic and the broader community.
  • Assisting clients with transportation and housing resources.
  • Guiding clients through financial benefit programs.

Collaboration with a multidisciplinary team, including licensed social workers, medical professionals, and nursing staff, is essential. The role requires accurate documentation and data entry within electronic medical records and tracking systems. Successful candidates will demonstrate strong multitasking abilities in a dynamic environment and will participate in required training and meetings.

Education and Experience Requirements:

A Bachelor's degree in a Human Services discipline is required, with coursework relevant to the position. Certification as a Clinical Social Worker may be necessary for certain roles.

Preferred Qualifications:

Preferred candidates will possess certification in case management and experience working with clinic populations, electronic medical records, and data management.

Position Requirements:

This position is based in a clinical setting, with occasional overtime as needed.
