Overview: This position involves delivering coordinated healthcare services to participants from special populations at heightened risk for HIV. The role includes supporting Care Teams by managing care for participants who are not regularly scheduled and facilitating integrated and personalized care planning.

  • Educates and orients participants and their families.
  • Oversees a manageable case load of participants.
  • Engages in training sessions and compliance audits.
  • Contributes to outreach and engagement strategies.
  • Administers HIV testing and ongoing monitoring.
  • Records and maintains treatment plans.
  • Involves in care planning and educational initiatives.
  • Collaborates with healthcare professionals and community organizations.
  • Participates in quality enhancement projects.
  • Completes additional responsibilities as assigned.
  • Availability for Saturday shifts may be necessary.

Requirements:

  • Education: A Bachelor's degree is mandatory.
  • Experience: Minimum of two years working with at-risk populations and/or Medicaid beneficiaries.
  • Skills: Proficient in computer applications and effective communication with callers.
  • Language Proficiency: Capable of drafting reports and presenting information clearly.
  • Analytical Skills: Ability to apply logical reasoning to address challenges.
