Job Summary

We are seeking a highly skilled and compassionate Social Worker to join our team at Cape Cod Healthcare Inc. As a Social Worker, you will play a vital role in providing exceptional care and support to our patients and residents.

Key Responsibilities
  • Conduct comprehensive assessments to identify patient/resident needs and develop personalized care plans.
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and effective discharge planning.
  • Refer patients/residents to community resources and organizations to support their needs.
  • Complete the Social Service section of the MDS assessment within the required timeframe.
  • Assist patients/residents with advanced care planning and involve them in the care process.
  • Facilitate communication with post-discharge residents to ensure a smooth transition to community care.
  • Ensure timely and informative social service progress notes in accordance with Department of Public Health and CMS regulations.
  • Participate in facility committees and educate residents and staff on their rights.
  • Advocate for patients/residents as needed and communicate with the Ombudsman Representative.
  • Maintain confidentiality of resident care information and adhere to Resident's rights.
Requirements
  • Bachelor's degree in social work or a related field from an accredited college or university.
  • Must be licensed in Massachusetts.
  • Minimum one year of supervised social work experience in a healthcare setting working with the elderly or disabled.
  • Member in good standing with the National Association of Social Workers and Academy of Certified Social Workers, Inc. (preferred).
  • Ability to read, write, speak, and understand the English language.
  • Knowledge of community resources.
  • Ability to make independent decisions and demonstrate knowledge and skills necessary to provide care appropriate to the age-related needs of residents.
  • Supportive team member with excellent communication and interpersonal skills.
