The Opportunity

We are seeking a highly skilled and compassionate Field Care Coordinator to join our team. As a Field Care Coordinator, you will play a critical role in supporting our patients with care outside of the traditional clinic setting. You will work closely with patients, families, and healthcare teams to provide personalized care and support, and help patients navigate the healthcare system.

Key Responsibilities:

  • Conduct standard patient assessments and documentation to facilitate a complete visit by the treating provider in the home.
  • Provide clinical support, including performing basic vitals and clinical assessments, and assisting with extended scope duties such as clinician-guided physical exams and health status screenings.
  • Coordinate care with patients, families, and other outside care teams, including specialists, home health, and community resources.
  • Assist patients and families in navigating the healthcare system, accessing community resources, and completing applications for benefits and services.
  • Participate in daily operations, including answering phone calls, responding to emails, and facilitating transitions of care services.
  • Teach patients and families to use technology to connect with their care teams and access healthcare services.
Requirements:
  • At least 1 year of experience working in home-based healthcare services or senior living care settings, or Emergency Medical Technician (EMT) experience.
  • Certified Medical Assistant (MA) and Basic Life Support (BLS) certification preferred or ability to obtain within 6 months of hire.
  • Strong written and verbal communication skills, with proficiency in computer technology and video platforms.
  • A proven track record of building rapport and trust with patients, and delivering individualized, human-centered care.
  • Clinical skills in performing basic vitals and clinical assessments, and experience facilitating access to community resources or social service agencies.
  • Ability to travel throughout the day to patient homes in the Seattle area, with mileage reimbursement provided.
What We Offer:
  • Competitive hourly rate of $26.75 to $27.75 per hour.
  • A comprehensive benefits package, including medical, dental, and vision plans, and paid time off.
  • Opportunities for professional growth and development, including training and education programs.
  • A supportive and collaborative work environment, with a team of dedicated healthcare professionals.
