Key Responsibilities
  • Manage a designated caseload as specified by Care Management Leadership.
  • Ensure compliance with all contractual obligations and care management standards.
  • Assist in the development and implementation of disease management and population health initiatives for high-need members.
  • Conduct outreach and in-person visits to assess and address the needs of enrollees in various settings.
  • Evaluate enrollees upon enrollment and at regular intervals to identify barriers and enhance care.
  • Monitor utilization patterns and take necessary actions in accordance with organizational policies.
  • Collaborate with healthcare providers to create and update individualized care coordination plans.
  • Document interactions and findings accurately and in a timely manner, adhering to company policies.
  • Participate in discharge planning and contribute to the transition of care for enrollees.
  • Communicate effectively with enrollees, families, and multidisciplinary teams.
  • Support enrollees and caregivers in understanding and complying with health services.
  • Facilitate scheduling and compliance monitoring for medical and mental health appointments.
  • Stay informed about best practices in care coordination and management.
  • Present accurate information about the organization to stakeholders.
  • Maintain a professional demeanor in all interactions.
Qualifications

Education:

  • Associate Degree required; Bachelor's Degree preferred.

Experience:

  • Minimum of two years in healthcare, case management, or related fields.

Licensure:

  • Must hold a valid Registered Nurse license in the District of Columbia.
