Position Title: Patient Care Coordination Specialist

Immediate Supervisor: Director/RN Supervisor

Exposure Risk: Limited exposure to bloodborne pathogens

Key Responsibilities

The Patient Care Coordination Specialist is tasked with overseeing patient visit arrangements and ensuring the accuracy of scheduling records and logs.

Key duties include:

  • Maintaining an up-to-date patient roster with essential details.
  • Updating scheduling logs and records regularly.
  • Preparing weekly schedules for field personnel and distributing copies accordingly.
  • Assisting in the coordination of services rendered to patients.
  • Monitoring and managing schedule modifications effectively.
  • Verifying compliance of completed visits against the master schedule and promptly addressing any discrepancies with the RN Supervisor.
  • Facilitating the referral process and preparing necessary documentation for the Payroll/Billing Manager and on-call coordinator.
  • Collecting weekly visit data and reporting findings to the RN Supervisor.
  • Communicating messages to field staff, office personnel, and community partners.
Work Environment

This position may involve stress related to meeting deadlines and is primarily office-based, requiring a combination of sitting, standing, and walking, along with significant telephone communication.

Travel may be necessary for local or out-of-town seminars, conferences, or meetings.

Minimal lifting of office materials is expected, and the ability to read standard print sizes is required.

Effective communication skills, both verbal and written, are essential.

Equipment Proficiency

The role necessitates proficiency in operating a PC, calculator, multi-line telephone, and other standard office equipment.

Company Insights

Access to patient records is a key responsibility, with discussions regarding these records occurring with management.

Qualifications

The ideal candidate will possess:

  • A high school diploma or equivalent.
  • Preferably, an LPN background in Home Care.
  • A minimum of two years of experience in a coordination role or a related healthcare field.
  • Strong organizational skills to manage office files, logs, and staff schedules effectively.
  • Basic secretarial skills and a courteous telephone demeanor.
