Position Summary:
The Client Care Coordinator role at Seabury Care Management entails delivering comprehensive care management services to clients. Key responsibilities include conducting client evaluations, developing care strategies, and providing ongoing support through home visits. This position necessitates timely documentation and reporting to the Care Management Supervisor. Client Care Coordinators operate remotely and are required to have their own equipment and means of transportation.
Department: Seabury Care Management
Primary Responsibilities:
- Conduct thorough client evaluations and develop tailored care strategies as per client agreements
- Meet care management service objectives based on work commitments
- Document service notes in the database within designated timelines
- Engage in outreach efforts to promote Care Management services
- Collaborate with fellow care coordinators and remain informed about community resources
- Participate in networking events as necessary
- Provide weekend and emergency support on a rotational basis
- Assist with coverage for colleagues during vacations and emergencies
- Perform additional duties as assigned
- Master's degree in social work with a current license in DC or Maryland
- Experience in working with older adults and their caregivers
- Proficient in Microsoft Office and case management software
- Excellent communication and organizational abilities
- Understanding of aging issues, local resources, and social work trends
- Willingness to travel within designated areas
- Strong problem-solving skills and leadership capabilities
- Liability insurance, valid driver's license, and car insurance
Physical Requirements: Ability to lift 20 pounds, operate a vehicle, and sit for extended periods
Equipment Required: Contractors must possess a personal computer, internet access, and a smartphone for field communication.
