Community of Hope is a mission-driven, innovative nonprofit organization dedicated to improving health and ending family homelessness in Washington, DC. We provide comprehensive healthcare, housing, and supportive services to low-income, underserved, and homeless individuals and families.

We are seeking a compassionate and dedicated Care Coordinator to join our team. The Care Coordinator will play a vital role in supporting our patients by coordinating their care, connecting them with resources, and advocating for their needs.

Responsibilities:
  • Develop and implement individualized care plans for patients based on their unique needs and goals.
  • Coordinate patient care across various healthcare settings, including medical, dental, behavioral health, and housing services.
  • Connect patients with community resources such as food banks, transportation assistance, and legal aid.
  • Advocate for patients' rights and needs within the healthcare system.
  • Provide ongoing support and guidance to patients throughout their care journey.
Qualifications:
  • Bachelor's degree in social work, healthcare administration, or a related field.
  • Experience working with underserved populations and/or individuals experiencing homelessness.
  • Strong communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Commitment to Community of Hope's mission and values.
