Job Overview

V-Tech Solutions, Inc. is seeking a dedicated Registered Nurse to enhance our client support services. The ideal candidate will be meticulous and committed to delivering exceptional care to individuals in need. Under the guidance of the Senior Care Manager, the RN Care Manager (CM) will play a crucial role in providing thorough care coordination for members with complex medical and behavioral requirements.

Key Responsibilities
  • Manage a designated caseload as specified by Care Management Leadership.
  • Ensure adherence to contractual obligations and care management standards.
  • Participate in the development and execution of disease management programs tailored for high-need members.
  • Conduct regular outreach and in-person visits to assess and address the needs of enrollees.
  • Evaluate enrollees upon enrollment and periodically to identify barriers and enhance care.
  • Monitor utilization patterns and take necessary actions in line with organizational policies.
  • Collaborate with healthcare providers to create and maintain individualized care plans for each enrollee.
  • Document interactions and findings accurately and promptly in accordance with company protocols.
  • Maintain confidentiality regarding enrollee information as per organizational policies.
  • Engage in discharge planning and ensure a smooth transition of care for enrollees.
  • Communicate effectively with enrollees and their families, promoting understanding and involvement in care.
  • Support enrollees in understanding the importance of health services and compliance with appointments.
  • Facilitate transitions of care across various healthcare settings.
  • Stay informed about evolving strategies in care coordination and management.
  • Exhibit professionalism and a customer-focused approach in all interactions.
Qualifications

Education:

  • Associate Degree required; Bachelor's Degree preferred.

Experience:

  • Minimum of two years in healthcare, case management, or related fields.

Licensure:

  • Registered Nurse license in the District of Columbia.
