Job Summary

We are seeking a highly skilled Care Coordination Liaison to join our team at Stanford Children's Health | Lucile Packard Children's Hospital Stanford. As a key member of our care coordination team, you will play a vital role in ensuring seamless patient care and experience.

Key Responsibilities
  • Act as a patient advocate, providing support and guidance to patients and families navigating the healthcare system.
  • Collaborate with healthcare providers, case managers, and social workers to identify and address barriers to care.
  • Develop and implement effective care plans, ensuring timely access to medical resources and services.
  • Coordinate with community agencies and organizations to provide patients with necessary support and referrals.
  • Supervise and train staff on care coordination best practices.
  • Participate in quality improvement initiatives to enhance patient care and experience.
Requirements
  • Bachelor's degree in a related field, such as healthcare administration or social work.
  • Three years of experience in care coordination, patient advocacy, or a related field.
Preferred Qualifications
  • Master's degree in a related field.
  • Experience working in a pediatric healthcare setting.
  • Knowledge of healthcare policies and regulations.
