A Brief Overview
The Cardiovascular Health Destination Service Line Patient Flow Coordinator, under general supervision and guidance of clinic manager and the program care team (registered nurse coordinators, advanced practice providers and attending physicians), coordinates and manages the day to day patient care coordination, transitions of care activities, and clinic operations of their designated program in the clinic. The scope of responsibilities includes patient flow management, resolution of complex work cell issues within the program, administrative and resource coordination for their assigned program and coordination of patient care for Cardiovascular Health Destination specialty programs in Cardiovascular Medicine, Cardiothoracic Surgery and Vascular Surgery. The PFC also collaborates with management to evaluate, recommend, and conduct performance improvements and workflow efficiencies within their scope of practice. This position serves as a standard across the Stanford Health Care enterprise for the Cardiovascular Health Destination Service Line.

What you will do
  • Under direction of a physician and/or resident physician, the focus of the job is to enhance the efficiency and improve the throughput of patients while maintaining and improving patient satisfaction. The Flow Coordinator (FC) will accomplish this in the following manner:
  • Record a physician-directed patient history of present illness (HPI), past medical history (PMH), family and/or social history and physical examination (PE), including pertinent positives and negatives.
  • Record procedures and the results of laboratory tests and radiographic images.
  • Improve the efficiency of patient care by assisting the physician with routine matters.
  • Serve as an ambassador/liaison between the physician, the patient, and the Cardiovascular Health staff. The FC may serve as a liaison between the physician and nurses, clinical associates.
  • At the physician's discretion, the FC shall accompany the supervising physician during a patient's history taking and examination. The physician will personally elicit the patient's HPI, which will be recorded by the FC.
  • The FC will record onto the electronic medical record (EMR) the details of the patient's visit as directed by the physician. The physician may elect to personally document any part of any patient's record, or to complete it in its entirety.
  • The FC can assist the physician with researching and recording a patient's PMH by assembling and reviewing nurse's notes, EMS reports, referral notes from other health care facilities, and hospital records (e.g., prior admission notes, discharge summaries, operative/procedure notes and records).
  • Must successfully complete Stanford's Physician Coordinator Training and Orientation Program and attend any required Stanford new-employee orientation sessions, complete all HIPAA/Compliance training and background/exclusion checks required, receive any vaccinations required, and sign all applicable confidentiality agreements and forms.
  • Performs patient care, including vital signs, weight, height, rooming and assisting providers with cardiovascular medication reconciliation
  • Performs 12 Lead EKG
  • Schedules, processes, prepares, and dispenses cardiac device (zio/holter) monitor and tracks patient remotely for results
  • Perform compression stockings measurements (Vascular & Cardiology)
  • Prep and assist with in-clinic Vascular Procedures including phlebectomy, vein ablation, varithena, sclerotherapy, Perm cath/TDC removal.
  • Assist with wound dressings for Vascular Surgery
  • Monitor and report patient's lab results, blood pressure logs and weight logs for cardiac medication management
  • Monitor, initiates and submits electronic prior authorization for cardiac medications
  • Schedules peer to peer insurance authorizations for cardiovascular clinic visits and cardiac and vascular diagnostics procedures/testing
  • Coordination of care for complex cardiac echocardiogram and vascular lab scheduling/surgery scheduling
  • Assist patient in coordination of procedure (i.e TAVR, catherization, cardioversions, pacemaker, vascular surgery, cardiothoracic surgery, transplant, etc.)
  • Understands proper use and applications for the following imaging modalities: radiology (CTA/PET, MRI), Echo, catheterization, carotid ultrasound, vein and arterial mapping.
  • Coordinates/triage cardiovascular hospital discharge appointments before discharge from inpatient
  • Assist, monitor and perform virtual rooming for cardiovascular providers
  • Weekly clinic preparation of cardiovascular clinic visit (i.e. pre-post op clinic appointments, lab work, monitoring and reporting results from heart device testing and diagnostic testing)
  • Coordinates day to day support operations and services for the clinic and cardiovascular program
  • Ensures the timely completion of work assignments.
  • Responsible for workplace safety and infection prevention within the scope of the role.
Education Qualifications
  • High School Diploma or GED equivalent AND
    Medical Assistant Certificate/Diploma from an approved school/institution or equivalent documented training/experience
Experience Qualifications
  • 1-3 years of directly related experience
Required Knowledge, Skills and Abilities

  • Knowledge of computer systems and software used in functional area.
  • Knowledge of applicable new technologies and ability to maintain and stay abreast of updates and changes.

  • BLS - Basic Life Support required

Base Pay Scale: Generally starting at $ $40.79 per hour
