Key Responsibilities
  • Act as a patient advocate, providing support and guidance to patients and families navigating the healthcare system.
  • Collaborate with healthcare providers, case managers, and social workers to identify and address patient needs.
  • Develop and implement effective care plans, ensuring timely access to medical care and resources.
  • Coordinate with community agencies and resources to provide comprehensive support to patients and families.
  • Supervise and train staff to ensure high-quality care coordination services.
  • Participate in quality improvement initiatives to enhance patient care and experience.
Requirements
  • Bachelor's degree in a related field, such as healthcare administration or social work.
  • Three (3) years of progressively responsible experience in care coordination or a related field.
Preferred Qualifications
  • Master's degree in a related field.
  • Experience working in a pediatric healthcare setting.
  • Knowledge of healthcare policies and regulations.
