Key Responsibilities- Act as a patient advocate, providing support and guidance to patients and families navigating the healthcare system.
- Collaborate with healthcare providers, case managers, and social workers to identify and address patient needs.
- Develop and implement effective care plans, ensuring timely access to medical care and resources.
- Coordinate with community agencies and resources to provide comprehensive support to patients and families.
- Supervise and train staff to ensure high-quality care coordination services.
- Participate in quality improvement initiatives to enhance patient care and experience.
- Bachelor's degree in a related field, such as healthcare administration or social work.
- Three (3) years of progressively responsible experience in care coordination or a related field.
- Master's degree in a related field.
- Experience working in a pediatric healthcare setting.
- Knowledge of healthcare policies and regulations.
