Nurse Coordinator

Stanford Health Care is seeking a skilled Nurse Coordinator to join our team. As a key member of our healthcare team, you will provide professional support, planning, and coordination of patient care services.
Key Responsibilities- Coordinate patient communication and education to ensure seamless care transitions.
- Collaborate with healthcare professionals to identify opportunities for improving patient care services.
- Assist in the development of operational needs and implement process improvements.
- Provide age-specific, quality patient-centered care using the nursing process and standards of nursing practice.
- Assess, triage, and provide nursing consultation, perform follow-up and care coordination during face-to-face encounters or using telecommunications technologies.
- Apply appropriate nursing interventions, such as identifying and clarifying patient needs, conducting health education, promoting patient advocacy, coordinating nursing and other health services, assisting patients to navigate the healthcare system, and evaluating patient outcomes.
- Focus on patient safety and quality of nursing care during each encounter.
- Interact with patients during face-to-face encounters or through various telecommunication strategies in the virtual environment.
- Act as partners, advocates, and advisors, assisting and supporting patients/families in the optimal management of their healthcare, respecting their culture and values, individual needs, health goals, and treatment preferences.
- Facilitate continuity of care using the nursing process, interprofessional collaboration, and coordination of and access to appropriate healthcare services and community resources across the care continuum.
- Collaborate with inpatient and other clinic staff to develop informational/educational materials appropriate for the patient population.
- Demonstrate leadership by inspiring others to achieve their best and acknowledging the positive contributions of staff in the manner they wish to receive it.
- Promote team building and participate in formal and informal development and evaluation of staff competency.
- Maximize productivity by focusing on goals, purpose, and priorities.
- Pursue lifelong learning to update and expand clinical, organizational, and professional knowledge, skills, and abilities in professional practice.
- Utilize performance improvement methodology to design data collection systems/processes and identify opportunities to improve care/service for patients.
- Bachelor's degree or equivalent from an accredited college/university.
- Two (2) years of progressively responsible and directly related registered nurse (RN) work experience.
- CA-RN (Registered Nurse) license and Basic Life Support (BLS) certification issued by the American Heart Association.
