Job Summary

We are seeking a highly skilled Clinical Operations Coordinator to join our team at Palo Alto Sub-Acute and Rehabilitation Center. The successful candidate will be responsible for coordinating and assisting with the completion and submission of accurate and timely patient assessments, in accordance with all applicable laws, regulations, and company policies.

Key Responsibilities
  • Coordinate and Assist with Patient Assessments: Ensure that patient assessments are completed and submitted accurately and on time, in accordance with all requirements and policies.
  • Report Changes in Patient Condition: Notify the Director of Nursing (DON) of any changes in a patient's condition.
  • Provide Education and Training: Educate direct care associates on patient documentation, updates, and changes, as well as training on Skilled Nursing Facility and other regulations.
  • Review and Validate Reports: Review Final Validation Reports to confirm that assessments have been completed and accepted into the CMS and other systems as necessary and prior to billing.
  • Collaborate with Business Office: Work with the Business Office to determine when to bill and when not to bill.
  • Review CMS Reports: Continuously review CMS Reports to address issues and improve processes to ensure that assessments are accurate and submitted on time.
  • Attend Meetings and Participate in Quality Improvement: Attend and participate in the Daily PPS Meeting and Monthly Triple Check to ensure that patient assessments are accurate and submitted on time.
  • Perform Functions of a Staff Nurse: Perform functions of a staff nurse when necessary.
  • Lead by Example: Lead by example in treating everyone at work kindly and professionally.
