Care Coordinator for Autism Services

Key Responsibilities
  • Facilitates effective collaboration, teamwork, and communication among healthcare professionals, community resources, and patients and their families.
  • Assists families and children diagnosed with Autism Spectrum Disorder by guiding them through the identification and navigation of available services and resources within the organization and the community.
  • Conducts community outreach and educational initiatives focused on Autism Spectrum Disorders.
  • Acts as an advocate for patients and their families.
  • Monitors and documents the interventions and outcomes for patients and families receiving Autism Navigator services.
  • Provides educational training for residents and clinicians.
  • Offers mental health counseling and support services.
General Requirements
  • Performs additional duties as required.
  • Adheres to all organizational policies and standards.
  • For detailed responsibilities, refer to departmental documentation provided during orientation.
  • Must comply with all regulations to securely maintain Protected Health Information (PHI) for patients. Annual training and adherence to the organization's Code of Conduct and policies are mandatory for appropriate PHI usage in the workplace.
Qualifications:
Educational Background
  • Master's Degree in Social Work is required.
Professional Experience
  • Preferred: 5+ years of experience working with children and families with developmental disabilities and Autism Spectrum Disorder.
  • Preferred: 5+ years of experience in providing guidance and education to parents, families, the medical community, and the general public.
Skills and Competencies
  • Must be a highly motivated individual capable of functioning independently.
  • Strong written, verbal, and interpersonal communication skills are essential.
  • Ability to engage effectively with various levels of the public sector and the medical community.
  • Excellent organizational skills are required.
  • Proficiency in using computers, fax machines, copiers, Microsoft Word, and Excel is necessary.
  • Ability to manage multiple tasks effectively in a fast-paced environment.
  • Must be able to prioritize and work efficiently under competing deadlines and changing workloads.
  • Flexibility in work hours is required to accommodate sudden changes in workload or job requirements.
Licenses and Certifications
  • Preferred: Licensed Social Worker (LSW).
  • Preferred: Licensed Independent Social Worker (LISW).
Physical Requirements
  • Occasional standing and walking.
  • Constant sitting.
  • Rarely lifting, carrying, pushing, or pulling up to 20 lbs.
  • Frequent repetitive motions and coordination between hands, eyes, and feet.
  • Constant talking and hearing.
