Position Overview

North Suffolk Community Services Inc. is seeking a Clinical Nurse Care Coordinator to enhance our Community Behavioral Health Center (CBHC) program. The Clinical Nurse Care Coordinator will serve as the primary point of contact for individuals pursuing Medication-Assisted Treatment for Opioid Use Disorder (MAT-PDOA) and will facilitate communication between patients and the MAT-PDOA medical team, including psychiatrists and psychiatric nurse practitioners, throughout the treatment journey.

The Clinical Nurse Care Coordinator will oversee the initial assessment process, ensuring that patients are appropriately evaluated for their opioid use disorder diagnosis. Following this, the Coordinator will assist patients during the induction phase, providing necessary support, education, and guidance under the supervision of the waivered medical provider. Regular contact will be maintained during the initial induction day to ensure a smooth transition.

Key Responsibilities:

  • Management of buprenorphine/naloxone and naltrexone assessment, induction, stabilization, maintenance, and relapse prevention.
  • Ensure compliance with state and federal regulations while collaborating with MAT providers, social workers, counselors, psychiatrists, pharmacies, and primary care professionals.
  • Facilitate communication between MAT providers and pharmacies, including medication history retrieval, prescription processing, and addressing insurance and safety concerns.

Qualifications:

  • Education: Must be a Registered Nurse with a valid Massachusetts nursing license. A Bachelor's Degree in Nursing is preferred.
  • Skills: Completion of an initial eight-hour Nurse Care Management training is required (may be completed upon hire). Experience in office-based treatment with buprenorphine/naloxone is essential.
  • Knowledge: Familiarity with treatment methodologies for severe mental illness and substance use disorders. Strong communication skills and the ability to work collaboratively within a multidisciplinary team are crucial.
