Overview:

The Healthcare Associates (HCA) division is a comprehensive adult primary care service at Beth Israel Deaconess Medical Center, committed to delivering extensive health services aimed at improving the quality of life for our patients. HCA caters to approximately 40,000 patients, facilitating over 200,000 patient interactions annually.

Position Summary:

As a professional registered nurse, you will employ the nursing process to provide direct care to patients and their families in a clinical environment.

Key Responsibilities:

  1. Assess the urgency of patient inquiries and manage telephone triage requests with clinical expertise.
  2. Gather relevant clinical information and utilize nursing knowledge to coordinate care during various patient interactions.
  3. Assist in scheduling both urgent and routine appointments as necessary.
  4. Collaborate with the healthcare team and primary care providers to manage patients with chronic conditions, including developing care plans and performing medication reconciliations.
  5. Work alongside LPNs and providers to ensure follow-up care post-emergency department visits and communicate effectively with patients regarding their care.
  6. Promote patient health through coordinated follow-up and education, supporting self-management goals.

Essential Duties:

Deliver direct patient care while making informed nursing decisions. You will be responsible for:

  1. Systematically evaluating the healthcare needs of individuals or groups and formulating, implementing, and assessing care plans.
  2. Coordinating patient care and directing support staff to ensure safe and effective patient-centered care.
  3. Building trusting relationships with patients and families, ensuring effective communication and advocacy.
  4. Meeting annual educational requirements, including emergency skills and unit-specific competencies.
  5. Engaging in professional development activities and quality improvement initiatives.

Required Qualifications:

Candidates must possess:

  1. A degree from an accredited nursing program.
  2. A valid Registered Nurse license.
  3. Basic Life Support certification.
  4. 1-3 years of relevant experience.
  5. Basic computer skills and familiarity with web-based applications.

Preferred Qualifications:

Experience in a teaching hospital and expertise in specialized areas is advantageous.
