We are seeking a skilled Care Coordinator to join our team at VITRA Health. As a Care Coordinator, you will play a vital role in supporting our clients in the Boston Region by providing comprehensive assistance to caregivers who care for friends and family in their own homes.

Key Responsibilities:

  • Conduct on-site visits with each client and caregiver to assess their needs and develop personalized care plans.
  • Obtain social histories and conduct psycho-social evaluations to identify potential risks and challenges.
  • Develop, implement, and review care plans in collaboration with our registered nurses and other healthcare professionals.
  • Train, evaluate, and supervise caregivers to ensure they have the necessary skills and support to provide high-quality care.
  • Participate in discharge and transition planning to ensure a smooth transition for our clients.
  • Review and document fire and safety procedures for each qualified setting.
  • Provide ongoing support with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
  • Coordinate social services with our personal care team to ensure our clients receive comprehensive support.
  • Participate in care plan creations and reviews, as well as discharge plans.
  • Have direct contact with clients, caregivers, and nurses to ensure seamless communication and care.
  • Manage all paperwork associated with Case Management, including admissions, referrals, visits, and medical equipment/social referrals.
  • Assist clients in finding community, financial, and legal services.
  • Review and assess caregivers' clothing, equipment, supplies, and other services.
  • Perform additional duties as required.
  • Participate in weekly clinical meetings in our Braintree office.
  • Conduct weekly calls to AFC caregivers to confirm compliance with program regulations.

Requirements:

  • Bachelor's Degree in human services, public health, or a related field.
  • Valid MA Driver's License and reliable transportation.
  • Fluency in both English and Spanish language (strongly preferred).
  • Proven working experience in case management or human services.
  • Excellent knowledge of case management principles, healthcare management, and reimbursement.
  • Previous experience with mental health aspects of care.
  • Strong communication, writing, organizational, and time management skills.
  • Solution-oriented with the ability to multitask.
  • Proficiency with the Microsoft Office Suite.
