Job Summary

The Health Services Registered Nurse, in the Home Care program, functions as part of the interdisciplinary case management team, and participates in service planning for Home Care consumers.

Key Responsibilities:

  • Conduct clinical assessments of consumers to determine their medical eligibility for programs funded by the Executive Office of Elder Affairs (EOEA) and the Division of Medical Assistance (DMA).
  • Perform in-home nursing assessments to determine the personal care needs of Home Care consumers.
  • Develop and implement care plans that address the physical, mental, and social needs of consumers.
  • Collaborate with interdisciplinary teams to ensure comprehensive care and support services.
  • Communicate effectively with consumers, families, and colleagues to ensure seamless care coordination.

Requirements:

  • Valid Registered Nurse's license from the Commonwealth of Massachusetts.
  • Bachelor's degree in Nursing or an Associate's degree in Nursing with at least one year of clinical experience.
  • Strong assessment, independent caseload management, and interpersonal skills.
  • Ability to communicate effectively and work collaboratively with consumers, families, and colleagues.
  • Bilingual preferred.

Work Environment:

Boston Senior Home Care is a private, nonprofit organization that provides case management, person-centered care planning, home care, and long-term services and supports to those who wish to remain in the community but require assistance to do so.

We pride ourselves on creating a healthy work-life balance for our employees, with all positions working Monday through Friday, 9:00 AM to 5:00 PM, with no weekend or night shift requirements.
