Position Summary:

Hours: Full-time, 8:30 AM – 5:00 PM
The Boston Health Care for the Homeless Program is looking for a full-time Registered Nurse Care Coordinator to join our Outreach Team. In this role, you will collaborate with an integrated, multidisciplinary team to reach out to individuals living in challenging circumstances throughout the city, providing both urgent and ongoing medical and behavioral health services. Success in this position requires a flexible, compassionate, and self-driven individual who aligns with our mission. You will be responsible for delivering high-quality nursing and care management in various non-traditional settings to a vulnerable population with a history of chronic homelessness.

Key Responsibilities:

  • Conduct nursing triage at a busy, low-barrier outpatient clinic.
  • Engage in street outreach, providing nursing care as needed.
  • Visit patients who have experienced homelessness in their residences to deliver nursing care.
  • Offer outreach-based care management and clinical support in collaboration with other team members.
  • Support patients dealing with substance use disorders and mental health issues, adhering to harm reduction and trauma-informed care principles.
  • Perform health assessments and create integrated care plans in partnership with patients and the primary care team.
  • Monitor appointments, referrals, and primary care activities, ensuring patients attend or reschedule missed appointments.
  • Participate in clinical quality improvement initiatives, documenting all encounters accurately and maintaining updated data.
  • Contribute to team meetings and case discussions.

Qualifications:

  • Current RN license in the Commonwealth of Massachusetts. A minimum of 2 years of nursing experience is preferred.
  • Familiarity with substance use disorders and behavioral health challenges.
  • Experience in care management is preferred; interest in working with underserved populations is a plus.

  • Hourly compensation begins at $35.22, with increases based on experience.
  • Full-time employees are eligible for a competitive time-off policy, including 4 weeks of vacation, health, dental, and vision insurance, a 403B retirement savings plan with employer contributions, and a pre-tax MBTA pass program offering a 40% discount. Additionally, eligible employees may receive annual increases and an extra $7,500 added to their base salary for demonstrated bilingual proficiency.
