Job Overview

Optimae Home Health Services is seeking a dedicated Referral Specialist to join our team in Des Moines. As a key member of our team, you will play a vital role in managing incoming referrals, fostering community relationships, and ensuring smooth coordination of care for our patients.

Key Responsibilities:

  • Manage incoming referrals from hospitals, clinics, and community partners, ensuring timely and appropriate care coordination.
  • Serve as a liaison between physicians, patients, families, and the care team to facilitate effective communication and care transitions.
  • Maintain strong relationships with community partners, physicians, and other referral sources to drive new referral relationships and growth.
  • Assist in the admission process by obtaining physician orders, completing necessary paperwork, and coordinating care transitions.
  • Coordinate hospitalizations, level-of-care changes, and patient discharges as needed, ensuring seamless care transitions.
  • Provide timely updates and follow-up on referral statuses to ensure transparency and accountability.
  • Support marketing and outreach initiatives by fostering new referral relationships and driving business growth.

Qualifications:

  • High school diploma or equivalent required; Bachelor's Degree in marketing, business administration, or a related field preferred.
  • 1-2 years of experience in marketing and sales, with experience in a healthcare setting preferred.
  • Excellent communication, interpersonal, and organizational skills, with the ability to work collaboratively with diverse teams.
  • Valid driver's license with a good driving record and reliable transportation.
  • Strong computer skills, including proficiency in Word and Excel.
  • Knowledge of HIPAA and healthcare regulations.
