Job Summary:

The Care Coordinator will be responsible for providing holistic care to our members, ensuring their needs are met and their goals are achieved. This role requires strong communication and interpersonal skills, as well as the ability to work effectively with various healthcare providers.

Key Responsibilities:

  • Assess and understand member concerns and whole health resources needed
  • Make and track referrals to ensure seamless care coordination
  • Follow up on referrals to ensure timely completion
  • Complete assessments and develop care plans to meet member goals
  • Coordinate care provided by behavioral, medical, and social service providers

Requirements:

  • Bachelor's Degree in Social Work or Human Services Field
  • Valid Driver's License and Insurance

Preferred Qualifications:

Bilingual a plus, but not required
