About This Role

UnityPoint Health is seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you will play a critical role in ensuring the seamless transition of patients through the healthcare system.

Key Responsibilities
  • Care Coordination
    • Screen and assess adult Medical Surgical In-patient and observation patients to identify needs and make referrals to appropriate multi-disciplinary services.
    • Prioritize patients for care coordination based on defined criteria.
    • Monitor and coordinate an interdisciplinary plan of care in partnership with the individual and their support services for needs and services across the healthcare continuum.
    • Assume accountability for the development and implementation of an effective discharge plan for complex care patients.
  • Discharge Planning
    • Collaborate with patients, caregivers, internal/external healthcare providers, agencies, and payers to plan and execute a safe discharge.
    • Identify and facilitate post-acute resource needs, including Home Care, Community-based Referrals, Diagnostic testing, Outpatient Therapies, Palliative Care, or Hospice.
    • Ensure that the patient's degree of vulnerability has been captured and documented on the Transitions of Care report.
  • Education
    • Optimize utilization of Healthwise for Patient Education.
    • Communicate patient/family learning needs to the direct care nurse and collaborate on education plans.
    • Refer to content experts as appropriate, such as wound care teams, Diabetic Educators, Respiratory Therapy, or PT.
  • Revenue Cycle
    • Demonstrate a working knowledge of financial and reimbursement processes to facilitate medical cost management.
    • Serve as a resource and educator to patients, families, staff, and physicians regarding financial aspects of individual patient resources.
Requirements
  • Bachelor of Arts/Science degree in a healthcare-related field or BSN preferred.
  • Two years of clinical experience in focused areas working with multidisciplinary teams.
  • Current RN Licensure in the state of residence.
  • Basic computer knowledge using word processing, spreadsheet, email, and web browser.
