About the Role

Halifax Health is seeking a skilled Licensed Practical Nurse (LPN) to join our Hospice Care team. As a key member of our interdisciplinary team, you will provide high-quality patient care to individuals with terminal illnesses and their families.

Key Responsibilities
  • Provide compassionate care to patients and their families, addressing their physical, emotional, and spiritual needs.
  • Conduct thorough assessments to identify patient needs and develop individualized care plans.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Administer medications and treatments as prescribed by physicians, while maintaining accurate records.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals.
  • Stay up-to-date with industry developments and best practices in hospice care.
Requirements
  • Current licensure as a Licensed Practical Nurse (LPN) in the State of Florida.
  • One year of experience in hospice, med-surg, or other acute care settings.
  • Valid driver's license and good driving record.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team environment.
