As an LPN, you will play a vital role in delivering personalized healthcare services, assisting with daily living activities, monitoring patient conditions, and collaborating with physicians and other healthcare professionals.
Responsibilities:- Provide direct patient care, including medication administration, wound care, and vital sign monitoring.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Educate patients and families on health conditions, medications, and self-care practices.
- Document patient assessments, interventions, and outcomes accurately and thoroughly.
- Maintain a safe and clean environment for patients.
- Communicate effectively with physicians, nurses, and other healthcare providers.
- Graduation from an accredited School of Practical Nursing.
- Current licensure as a Licensed Practical Nurse in the state of Florida.
- One or more years of experience in an acute/sub-acute care facility. Home health experience preferred.
- IV certification required.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a team.
- Valid driver's license and reliable transportation.
